Dated April 15, 1991, the document outlines Cobain's concern as "Epigastric Pain" and lists "Musician" as profession and "David Geffen" as his employer. A photocopy of the doctor's notes, dated April 17, is included with the document and shows that despite performing a battery of tests, the doctors could not conclusively determine the cause of Cobain's issue and that he was discharged accordingly.
