Signed in Middlesex County, Massachusetts, the consent form reads: "I, Michael Jordan, a citizen of the United States of American over 21 years of age, hereby authorize Nike, Inc., a corporation of Oregon, to use and register my name or any portion thereof as a trademark in the Austria under Application No. AM 2862/91."
