Nicknamed "The Golden Arm", Hall of Fame quarterback Johnny Unitas remains one of the most decorated players in professional football history. Owner of three NFL championships and a Super Bow LV title, Unitas would land four NFL MVP awards and 10 Pro Bowl selections. Unitas signed the top of this document, likely a survey sent to him by a fan, in black ink next to the word "Name:". The survey, which pinpoints questions about the 1959 and 1960 seasons, includes eight typed questions, each with a handwritten response from Unitas in black ink. They include, among others, "Football Honor of Which You're Most Proud," to which Unitas answered, "Jim Thorpe Trophy," and "Favorite Hobby off the Field," a question to which Unitas responded, "Any type of Sports Activity, Kids." The 8.5" x 11" white sheet of paper features five horizontal creases and one vertical crease from when the document was folded. All text on the document remains vivid and completely legible. Accompanied by a full Letter of Authenticity from PSA/DNA.
